Survey about medical care by seriously ill mind and body child with a disability, person

We carried out questionary survey to grasp the use situation of living condition of seriously ill mind and body child with a disability, person living in the city and family cared for, medical institution which visited a hospital for treatment and the welfare service in Yokohama-shi in 2016, and to make use for future measure promotion.

I sent questionnaire targeting at people who corresponded to all from 1 to 3 next.

It is person with resident registration in 1 Yokohama-shi
Where 2 certificates of the physically disabled (the limbs inconvenience) first grade or second grade is possessed
3 love that notebook (nursing notebook) A1 or A2 is possessed
