About personal information protection basic policy

We establish particularly important duty and positioning, the following basic policies (privacy policy) and, in our center, will act as information and protection of privacy to affect medical treatment of patient for protection of information of patient after common knowledge thorough to personnel.

[1] About basic way of thinking for personal information protection

  1. In our center, we observe purposes of (called "laws and ordinances" as follows) such as associated laws and ordinances commencing with "law about protection of personal information" or "the regulations about protection of Yokohama-shi personal information" and the regulations in protection of personal information of patient and act for certain protection of information. In addition, along guideline or guidelines that country and related organizations including "guidelines for the appropriate handling of personal information in medical care, care-related company" that Ministry of Health, Labour and Welfare devised in total established, we perform appropriate operation.
  2. In our center, we inspect the regularly contents about regime about protection of personal information of patient and the approach and plan continuous review and will act so that personal information of patient is permanently protected.

[2] About collection of personal information

In our center, we limit only to information of necessary range concerned with medical treatment in collection of personal information of patient and perform with means that is appropriate and fair about information provided by patient and other medical institutions.

[3] About offer to the use purpose of personal information and third party

  1. We use within the use purpose after having announced beforehand in our center unless it establishes in laws and ordinances about the use purpose. About public announcement of this use purpose, we perform through homepages in bulletin board in our center and the Internet.
  2. When we are going to perform the use not to be included in this use purpose newly, we shall have agreement of which can act for intention of patient or the person some other time.
  3. We do not provide personal information of patient to third party unless we set of laws and ordinances when we perform in conformity with authorized procedure in hospital after it was raised as the use purpose in our center without having agreement of which can act for intention of patient or the person beforehand. In addition, as a general rule, we put measures that cannot identify whether third party is information of patient unless we fix for laws and ordinances when we provide.

[4] About safety management measures

In our center, we carry out appropriate safety management measures for prevention such as leak, loss, damage or manipulation of personal information of patient. In addition, when education to prevent accident to be concerned with these personal information targeting at the staff of all our centers handling personal information of patient receives training regardless of the type of job, official post and rank, section and entrusts with desk work to be concerned with personal information, we oblige to similar measures for supplier of trust ahead and perform appropriate supervision.

[5] About disclosure, correction, suspension of personal information

In our center, we will cope based on request about disclosure, correction, suspension of individual contents of patient fixing for laws and ordinances quickly.

About the use purpose of personal information

In our center, we will tell the following purposes about having possibilities to use about personal information such as patient or family based on "the personal information protection basic policy" (PDF: 156KB) mentioned above.
We will manage according to related laws and ordinances commencing with "law about protection of personal information" or "the regulations about protection of Yokohama-shi personal information" in the handling of personal information that we advocated below appropriately.
